Item Description

Comments - 

Condition - The sheet has been folded in half as can be seen in the images. The more pronounce fold is horizontal but the sheet is a vertical fold as well - see the image where the two folds meet in the center of the sheet. 

The paper has a bit of over all light soiling. The edges appear to have gum slop over and there are small tears along the left side of the sheet some of which can be seen in the images. 

There are some markings in the upper left corner of the sheet. The three vertical dash lines appear to be in ink. I am not sure if the lettering is in ink or pencil. 

The upper left corner also has two large but light creases which can be seen in the image with the markings. The diagonal crease is hard to see but it runs just to the left of the three vertical dash lines.
